Media defect

A media defect is a read error when data, which has been correctly
written, cannot be recovered within 16 retries.
Any media defects are noted on the drive defect map. Locations are
noted by head, cylinder and bytes from Index.

ST251/ST252 Media Defect Specification
There will be no more than forty-two (42) defects total per drive.
Cylinders 0,1 and 2 are defect free.
